Rates & Insurance
Melanie Swain is a fee-for-service provider. She is a Licensed Clinical Social Worker in Pennsylvania and Massachusetts and may qualify as an out-of-network provider. Services may be covered in full or in part by your health insurance or employee benefit plan.
Please check your coverage carefully by asking the following questions:
- Do I have out-of-network behavioral or mental health insurance benefits?
- What is my out-of-network deductible and has it been met?
- How many sessions per year does my health insurance cover?
- What is the coverage amount per therapy session?
- Is approval required from my primary care physician?
Payment
Payment is based on 55 minute sessions. Please contact for pricing.
Cash, check and Visa, MasterCard, American Express or Discover accepted for payment. Payment is due at the time of service.
Cancellation Policy
Please call as soon as possible if you need to cancel or reschedule your appointment. If you do not show up for your scheduled therapy appointment, and you have not notified me at least 48 hours in advance, you will be required to pay a no show fee. If you have an emergency, a late cancellation may be rescheduled if another appointment time is available.
